Cancellation of HealthInsurance

Respected Sir,

I am vanisree. J working in SATYAM .I got two credit cards and without specifying any details about health insurance they conform my helath Insurance scheme.

So now i want to cancel the health insurance. I made call to helpline several times but i didn't get proper information from them. So kindly accept my request and cancel my Health scheme.

One of the credit card no for which i got the bill is:4477XXXX4004
