Never applied n never received card

Hi

i am jagdish thakker i am regular payer and my account status is very good i have never been defaulter in any bank or any transection or never retun ant cheques since last 15 years

my problem is somebudy has apply on my behalf with wrong adress like street name is devji ratansey marg he put tatansey marg instead of danabunder panabunder n pin code 400009 he put 400900 or 400090 it was totaly wrong card was issued by icici bank in feb i received first bill in octoberi have wrote said bank 20 times but bank has put me in defaulter list with you now i am facing dificulties for getting loan or new credit cards for what i have not done i assure you that i have never apply or never received said card and still i am platinium card holder of same bank if you can help me then ok or please advise me what to do there is no option left

can i move to consumer court or any court please advise me

for your ref my date of birth is 29/02/1968

my resi adress is a 901 lotus hillview chs ltd model town bal rajeshwar road mulund west mumbai 400 080

and my office adress is 79 ashok chambers devji ratansey marg danabunder mumbai 400009

my pan no is aacpt5777d
