Carelessness by passport department to get processes

Dear Sir/Madam

Greetings!

I applied for renewal of my passport which got expired. The application was submitted on 1st May. I got one of communication on 5th Aug that passport office required marriage certificate as per Annexure-D of the Passport information booklet attached to Form I, while I am Male and Male not supposes to fill required form. Also, I declare that I have never changed my name in the past and present. I have sent e-mail to respective PRO about this (Copy enclosed here-with for your reference) but yet not to receive any response on the subject and since then I'm trying to contact the passport department over phone and email to know about the status but none of them bothered to reply.

I visited their online status page and found there is same status available for my Passport application. My Passport Application details are:
