Regarding not received Proper EPIC

Respected Sir/Madam,

I would like to say that I have shifted my residence from B-68, Najafgarh Park New Delhi to B-89/A, Najafgarh Park, New Delhi-43 under AC-35-Najafgarh. I have filled the form -8 issued by Election Comission and my address has been changed in current voter list but My old EPIC No. DL/03/028/045396 exists till now in which my address is given B-68, Najafgarh Park New Delhi.

I would like to add that correction in name and age also required.

So you are requested to take necessary action in this regard and to correct the same and arrange to issue the new EPIC of my family alongwith me.
