Doctor do not give me Bill for mediclaim

I am Rajiv Residence of Gurgaon, ...I suffered with Denguee Fever from 2 Sep... I went through the Treatment From Rajdhani Medi clinic-New Colony Gurgaon for 3 to 4 days...

The doctor Named was Dr. Baweja... He reffer me to ARTIMS Hospital, when the my blod platelets reach burst condition. But anyway, i treated from MAX Hospital gurgaon... and get well back...

Now i need to claim for mediclaim rebursment, but Dr Baweja-Rajdhani medicalCenter is not in fever to give me Bill of my treatment his clinic...
