Company not issuing PF Form to be filled

I am an Ex-Employee of eFunds International India Pvt. Ltd (Recently Acquired by Fidelity National Information Services I was emplyed there in the year 2007 for a period of 8 months

I have completed all the exit formalities however they are reluctant to relase the PF form to me.

They claim that I owe the company an amount (which is more than the salary I got from them) However in this case let me mention I had provided them with the exact required notice period of 30days, which they claim not to have recieved, and very convienietly to them the concerned person who may have recieved it had left the organisation too at the same time

Also as company mentioned in the offer letter about mediclaims, which I had duly submitted at the time of employment, was not issued to me for reason of not listed sickness or disease.(heart).

So I am not able to claim the mediclaim as well as my PF

I do not care about the mediclaim but i would very much want to claim my PF.

Kindly advice.
