My name not included in VOTER LIST

I have submitted Form 6 several times to include in voter list (for myself and my wife). Finally my wife's name included in voter list and my name not included in the list. I have submitted all necessary evidences (ration card copy) to prove my residence where I presently living. I hv all original documents like PAN CARD, DRIVING LICENCE, PASSPORT, TELEPHONE BILL, RATION CARD etc. in the same address where I am presently living. What else I need to present to the Govt. to include my name in voter list??? I submitted Form 6 several times from the same address where I am residing. Why didn't they include my name till now. Why didn't they inform me the reason of not including my name in voter list???
