Transfer of voter id card

I have shifted from "8 Hemendra Sen Street To 26/1, Dr Nilmoni Sarkar Street". i have voter id card but my name is not avalable in voter list at my old address.

i want a fresh voter card at my present address.

what to do and how do please reply in details.
