Name & Address wrongly spelt in Voter ID card

Dear Sir/Madam,

My Voter ID card No. is: XPD1065275.

In my voter ID card the spelling of my name, my Fathers name and my address has been wrongly written. Except my photograph nothing is correct in my voter ID card.

Please issue a new card or guide me how to get a new one.
