Non issue of voter id card in new name

Sir,

i got my name changed from sujatha to vijayavarnikkha in the year 2005. and i reside at g-4 Bora apartment, 52. kothaval chavady street, saidapet, chennai-15. My father name is N.EKAMBARAM, MY voter's identity card no is TN/03/014/0381406 bearing my old name sujatha and issued on 22/08/1998 The name change had been notified to the concerned authorities, and my new name is enlisted in the voter's list, but the new name is not included in the identity card. what is the procedure to change the name? shall I get a new identity card with the same id card no? or with some other identity no? How to get the same? please help me to get my identity card soon?
