Voting I-d card sent for correction received afresh with additional mistakes

The I-D Card No MT/08/036/478803 was sent for correction in the spelling of my father's name, but received new card no ROL0457954 with additional mistakes. Details as:

1) Another ladies photo affixed.

2) Father's name not rectified ie Harish is still wrongly spelt as Haresh.

3) Surname wrongly written as Bhojvani insted of Bhojwani.

4) Father's name written as husband's name.

5) Date of birth written as XX/XX/1975 instead of 15/07/1976.

6) addition information is added as"surrounding locality" which is not mention in the request form.
