Non payment of Pension

Dear Sir,

My employee family pension payment order is UP/NOI/2030. I am alligible for pension with effect from 29.05.2008. But I have received pension of only two months @ Rs. 720 per month till date. As advised by Axis Bank I had already submitted survival certificate in May. But when I enquired from office in August I was told that it has not been received. I have again got issued survival certificate by Axis Bank duly signed by me and bank has sent the same to your office on 05 Sept. You are requested to kindly release all the pension amount with effect from 29 May along with arrears and oblige.
