Dear Sir /Madam

This is to inform you that Employee Provident Fund Organisation, Regional Office had issued the Cheques of SBI for my PF amount settlement to the ICICI Bank, Rajkot where my Saving A/c is there.

Now the ICICI bank has returned the cheques to me at my residence address as cheques are issued in the name of “KANJIBHAI SHINGADIYA” in stead of “ROHIT SHINGADIYA”.

The printing mistake is either done by ICFAI or Employee Provident Fund Organisation. I had applied for my PF amount in the month of September. I was continuously following this case and finally after 8 months I have received cheques with difference in payee's name which has frustrated me.

I kindly request your good self to please reissue the cheques in the name of Mr. Rohit Shingadiya with ICICI Bank Account No 015301514557

Further I would be eligible for the Interest of 8.50% on my total PF amount for whatever delay period has occurred.

I request all of you to provide your kind co-operation to bring the immediate solution of above matter.
