Provident Fund not received

Respected Sir/Madam,

I had surrendered my form for my Provident Fund claim in June I was told that would receive the amount within 6 months to my bank account, (bank account details given). Now 11 months to 1 year is over and I have not yet received my dues. Kindly check and let me know, details given below.
