EPF not yet transferred since 2008

This is with reference to the transfer of my EPS account from MH/KAN/45805/43 to MH/4544/05947. I would like to state that the Form No.13 (Application for transfer of E P F Account) was submitted on 5th November, to the Regional Provident Fund Commissioner's office at Bhavishya Nidhi Bhavan, Charkop, Kandivli, Mumbai. However, till date the transfer has not yet taken place even after more than 2&1/2 years. Request urgent help in the matter.
