Dear Sirs,

With reference to the above subject, please note that I have submitted the withdrawal form to Regional Provident Fund Commissioner, Shastri Nagar, Meerut, UP on 08.09.2005 but till date I have not received my amount. I have tried hard to withdraw my amount but failed to receive in my Bank.

Please note that I have already left Delhi job since 2005 & have joined a private company in Ahmedabad, Gujarat. I am finding difficult to come to Meerut for the withdrawal. Also please note that my given bank account no. (ICICI Bank, Prit Vihar) has been closed since 2006. Presently my current bank is SBI, HDFC Bank.

Request you to kindly look into the matter & help me in withdrawal.
