PF Transfer not affected since 2003

Dear Sir/Madam,

I, Munish Kesharwani, use to work for Wilco International Systems Pvt Ltd and then shifted to Bangalore to work for Tavant Technologies India Pvt Ltd. I requested transfer of accumulations on my PF account number KN/34284/28 (which is maintained in EPFO Bommanahalli Bangalore) on November from PF account number AP/HY/35709/39. I am told that the PF Amount has been transferred to Bangalore however the same has not been affected in my PF Account citing lack of Annexure K document. I requested PF Office Hyderabad (which maintained my previous PF account) to issue a copy of Annexure K, enabling the transfer & accumulation to my PF Account. PF Office Hyderabad said that they've resent the Annexure K. However PF Office Bangalore continues to maintain the status that they've not yet received annexure K.

What should be the next step? I request your help, guidance and suggestion please.
