PF claims return

I was working in Private company and resigned due my health problem. I sent the PF claim for my account No. 39959/56 claim through the company in January. Before submission I went to PRO/EPFO, Rayapettah office clarify all the points. Based on the discussion the claim was submitted.

After one and half month the claim returned due to some reason. Then we went and clairy with PRO/EPFO chennai and submitted again.

Again it is returned by EPFO/Chennai. I need know why is happaning like this. I do not know what they want? or the officials are not having knowledge about EPF claim?
