Ight issue of shares

I having shares of Fact enterprises Ltd (ISIN: INE923D01019) which the i have bought before the ex-date of right issue declared this financial year. This shares are lying in my D P account 1202390000213341, D P Name Cochin Stock Brokers Ltd. Mean while i have not received the right issue form for applying the same and at the same time not credited my account with the shares of right issue. Currently the value of shares has been decreased in proportionate with the allotment of right issue shares.
