Wrong name on id and want to reissue the card

Sir,

Iam having voter card made in the name of girij kuti which was wrongly made, the actual name is mrs. g. girija w/o mr. s.g.kutty and also the address given on the card is not proper so that I want to re issue the card regarding which i went to the concern area dept but they are not giving any proper response and said that my name has been deleted from the list. kindly help me out in this matter.
