Sub: Smt. K.A. Mangamma, MO, PHC, A.S. Pet, GPF A/c No. 15945/PH – Final Payment of Rs. 1, 29, 844/- - Not received.

I would like to beg your help in getting my GPF amount of R. 1, 29, 844/-, the necessary documents in regard was already sent to all the concerned departments years back.

I would like to inform that I have retired from service on 30-09 from Primary Health Centre, Anumasamudram Peta, Nellore District, Andhra Pradesh.
