Voter ID Correction

Dear Sir/Madam,

Please take this complain or request as priority. I am Radhakrushna Tripathy, My Election ID no is: ZUX0231977 from 143 Jeypore Assembly Constituency Orissa. I got my voter ID card in the last year after applying for 2 to 3 times that also not correctly. My date of birth is 27/03/1988 but in the voter ID they have mentioned as 27/03/1987 and my proper address is At/Po: Kumuli, Via: Bhairabsingh Pur, Dist: Koraput, Orissa, but they mentioned it as Kumuliput. I have the voter ID but there is no use at all. I have given the correction request form also but there is no response till now. I have to apply for my Passport and Driving Licence which I am unable to apply for. Due to this I face a lot of problem and don't know till when I have to face it. So its my humble request to do the needful.. Please take a necessary step against this.
