Form16 & PF amount not received

Dear Sir/ madam

I was employee of above said company till April 04. My employee no. is S06369, PF a/c no. 39111. Till date i have not received my form 16 as income tax was debited from my salary, so i need it to fill ITR. Again i do'nt received any PF amount which was debited from my salary during service period. when i checked it with EPF websites, it shows that claim was not received by company i.e By HBL Global Pvt. Ltd. while i have allready submitted all required documents to my employer by post.
