PF not received

Dear Sir/Madam,

This is regarding my pending relieving with HDFC Bank (erstwhile Centurion Bank of Punjab Ltd.) I had resigned from Bank’s services on 16.02.2008 and submitted my relieving kit, duly signed by Branch Head and Cluster Head, to Ludhiana regional Office, and as per my last communication with Ludhiana regional Office – HR Dept, the same was forwarded to Bangalore Office of the Bank. But when I contacted the HR Dept at Bangalore, I was informed that the same kit had been lost in transit.

I was then asked to fill in the format for calculation of the dues, which was forwarded by Bangalore Office. I had forwarded the same, duly filled in, back to Bangalore Office but till date, I have not received either my relieving or my Provident Fund amount.

As I have migrated to Canada you are requested to courier PF amount to my residential address along with my relieving letter at the earliest as the same is required by me for further submission in Embassy. My visa will be extended only on the basis of letter and the last date for submission is 20th December

You are requested to please expedite the matter.
