Staff provides wrong information while selling its products

Sir

my self vidhu sharma and i have opened an account having acc no-03021000113364.when the staff member of the hdfc provides me a gold card at the tme of account openening and asked that we will not deduct any charge from you and after opening of account i have found that balance is deducted 484 for gold debit card.now i want to close my account and i tick mark on the column-5 (unsatisfied with staff) the staff member refused to close my account.plz close my account and take appropriate action and return my 484 rupees deducted by wrong information.
