Dear sir,

I received a letter from you dated 12.02.03 Version : 2.11 Re. Master Policy No: GSCA / 10431 informing me about the commencement of payment of Annuity due to me vide Annuity No. 091Y0501063097.

Amount Payable to Niminee: Rs. 17133.00

Commuted value: Rs. 8566.00 Received via cheque no. 397879 dated 20.02.03.

Also it is mentioned in the letter that the First Annuity Instalment was Due on 01.05.2003 amount Rs. 1158.00.

I have a saving account in Bank of Maharashtra, Sayani Road Branch, Mumbai. (A/c No. 26150)

As I was relocated to Ahmedabad I did not get opportunity to check my account with the bank. Yesterday I got my Pass book updated and was surprised that there is no such amount deposited with the Bank. As per calculation I have still to receive Rs. 8567.00 as Annuity Amount.

I request your goodself to please look into the matter and see that I receive the remaining amount of Rs. 8567.00 as six years have passed. Please look into the matter urgently.
