ECS instructions not implemented & Poor Response

Sir,
This refers to my home loan A/c No. 10024692681. I received an application for standing instructions of paying my EMI through ECS (Electronic Clearing Service). I filled the form got it verified from my bank (ICICI) and send it same to your Office (Retail Assets Centralized Processing Centre (RACPC), State Bank of India Bhavan, 1st Floor, Bandra Kurla Complex, Bandra (E), Mumbai - 400 051 through Vichare Courriers on 27th July. Even after 2.5 months, I have found that it is not been implemented.
Secondly on the application form, It has been mentioned in bold letters to contact Sanjay Mishal (022-67514159) and Mamta (022-67514130) for any assistance. I have tried this numbers atleast 100 of times. Sanjay Mishal's number keeps on ringing and Mamta's number is always engaged. I also tried the cell number of Sanjay Mishal (9890238890), that too was switched off. I tried other numbers and tried to pass on the message to Sanjay Mishal Via. the person who picked the phone.
I am surpised and shocked to see the type of working by such a reputed bank in such a competetive market. I have only left with the option of requesting somebody senior to look into this matter and send me the feedback.
