Regarding voter id issuance problem

Respected Sir,

I am a house wife, and i have applied for new Voter ID in cuddalore, I have given Form 6 applications along with all the address proof (#15, Vanathi Nagar, Koothapakkam, Cuddalore - 607 002) for two times and people came for verification for both the times, and in the voter ID list, my name has not been included.
