Complaint regarding card ending with 1504

Abn Amro has issued a card to NEETU MISHRA in nov 06 for one year. It issued a fresh card which was not deliverd. Then they charged membership fee. I have no idea of the card no. Its last four digit was 1504 Add. was D-136 2nd floor sector -40 Noida 201301

I want full information and the proof of card delievry for which they are harassing me on regular occasion
