They charged wrong outstanding bill

I was credit card holder of HSBC Bank.
When card is issued from your bank, after few days i received call from TATA AIG Insurance and they issued me insurance for 3 months premium was 10.00 (interest free) monthly. But I never received any insurance documents from TATA AIG regarding insurance cover. This can be verified through the conversation recording between me and TATA AIG executive.
They have not any receipt or evidence of regarding my receiving of the insurance document. When i received first time statement from bank, I talked with bank customer care executive and she has recorded my request and assured me to roll back the amount and told me to this will be reflected in next statement. I was waiting for your next statement but i never received any statement from bank.
But few days back, I got SMS form bank regarding this out standing bill. I want to know that regarding insurance my premium is 10.00/month then how this large amount bill comes in my favor. According to me it should be 30 Rs. only.
As per customer care Excutive and mail from bank, My account is already closed on 17 nov. But Now i got new sms from bank as oustanding bill is 3700.00.
I have never used / swapped this card. Please resolve my problem.
