I have not received my PAN card

Sir/madam

i have not yet received my PAN card. i have made many complaints regarding this but no use. i personally went to UTI Technology services, Hyderabad and made a complaint against my PAN. but they asked me to pay Rs-67 again to send my PAN to me. I have paid it. but i have not received my PAN. After some days i again went to the office and asked about my PAN. They said we have already sent it. The funny thing is they are showing me the old acknowledgment when they sent very first time. i asked about it but they are not responding properly. what should i do. my PAN details are as follows.

i went to post office and made an inquiry about my PAN with the UCP no: they told there is no name and such number.

i have made many complaints to the UTI authorities but no use. please take proper actions and help me to get my PAN.
