PF form lost by PF kandivali staffs

My PF form lost by PF kandivali staffs.

now they say that fill up the new form again.

what the funn they are doin with my form... they simply lost it..

there are lot of qualified peopple looking for job in recession. so i suggest give them the job & remove PF staff who are not interested to do their job well.
