Non-receipt of PF settlement dues

Subject: Non-receipt of PF settlement dues

Reference: PF No. MH/BAN/47808/1237.

I left the job from ICICI Bank in November and submitted my duly signed PF forms vide my letter dated 04.02.2008 for settlement of my dues and subsequent reminders dated 10.06.2008 and 19.12.2008.

I have not received any communication from Regional Provident Fund Commissioner, Mumbai rearding the above case but nothing has been received from them till date.

I request that may PF settlement dues may kindly be transferred to :
