Efund of excess payment of Policy No.415867230 not credited

I had made online payment of LIC Premiun on 26/1/09. Due to site problem I had tried three times on 26/1/09 & two times on 27/1/09. But ICICI Bank had debited my S/B A/C No. 624601061654 five times on 27/1/09.When I made complaint to Bank they told to me that they had five times payment made to LIC of India sucessfully. Then I lodged complaint to LIC of India. They mailed me reply that we received only two payments. Out of which one adjusted with premium & issued premium receipt and another refunded to bank. I got premium receipt of one payment. Then I complaint to ICICI Bank but they are not giving proper answer and also not credited to my account
