Wrong Late payment charged in OD Account

I had been charged two times Late payment penalty without receiving any statement.

This is ridicolous.

In the last month the penalty was charged even after paying the minimum due amount.

I am not sure what to do? I have already register a complain regarding this to account manager but not getting any assistance.
