Deduction of money for not issued debit card

Dear sir,

I have received the bank statement for the quarter ending june wherein the Bank has debited an amount of Rs 111/-(Rs one hundred eleven) from my bank a/c towards debit card charges. I would like to bring to your kind notice that, I DO NOT HAVE ANY DEBIT CARD OF THE bANK.The particulars of my BANK A/C ARE AS UNDER -

Savings bank a/c no 003901227763

Branch- ICICI BANK, SHIVAJI NAGAR BRANCH, GHOLE ROAD, PUNE-41100

KINDLY DO THE NEEDFUL AND CREDIT THE AMOUNT DEDUCTED IN MY SB A/C at the earliest.
