Money discrepancy

I have withdraw Rs.400 form ATM of icici bank at neheru Place branch in 19-may, but there are deduct 5000 extra amount from my account (Account no.071601501289) untill i did not get 5000 amount, when i complaint to customer care they said for inquiry after some time they send me message that we can't able to retun your amount, when i said him to give me proof of withdraw and show clips they said to contact to Branch manager of your branch, when i go branch office, branch manager said that i will give you proof within a week but now up over more that 12 days, i also have complain to icici customer care which is provided by icici bank and it divide at stge level, i have complain to Customer service (Mr. Avijit Saha), Mr. Shiva Kumar Tadikonda (Nodal Officer), Mr. B.Madhivanan, Senior General Manager, Service Quality, but get only complain acknowledgement mail and reference number '034-239-427' on May 23 and get another mail by their executive of Mr. Avijit Saha, for wait 3 days, after 3 days i again send mail him then they reply give me more time but still get nothing response from him, i again complain to Mr. B. Madhivanan but now they don't reply me.
