Shocking Bank statement

I Recently Recieved Bank Statement from ICICI Bank which shows that 5000 Rs Cash was withdrawn on 14 Sep from ATM which is wrong because i did not used ATM and my ATM CARD was with me inside locker nobody knows the password, i dont know how it is possible to Debit 5000 Rs amount from my account through ATM/CASH without i using it.

I Complained to ICICI and they say to register FIR but how did simply 5000 Rs was withdrawn when my ATM was safely with me i cannot understand and i have not used ATM for so many months. Could there be mistake on the part of ICICI.
