Cancellation of Credit card

Dear Sir/Madam,

My name is Rangaja S.V.I have a salary account in ICICI bank. Based on that they issued a Credit card without my concern. In the credit card the name is printed as RANGARAJAN S.V.After that they issued Lombard Health card based on credit card, they charged the amount in the credit card all without my concern. I complain to customer care about the mistake in the name and do the needful but there is no response from that side. I haven't used the card not a single time.

Then, i cut the card with covering letter for cancelling the card and drop in the drop in box. Now i received the statement for payment of the credit card therough courier & also a call from the customer care that i have to pay the payment otherwise my name will come under defaulter payment list & i can't apply for any other credit card in India.

I was mentally wounded by this statement. I don't know what to do for this. Pls help to solve the problem.
