B – 110, IInd Floor, Near Ram Mandir, Vivek Vihar Delhi – 92 Have A ICICI Lombard Health Insurance Against Policy No. 4034/FPP/03518169/00/000 & Card No. IHPN- 03672945/01. I Was Admitted In Hospital (Deepak Memorial Hospital) Suffering From Dengue Fever & Hospitalized from 25/10/08 to 01/11/08. As Per The Clause Mentioned In Your Policy Total Days Of Hospitalized the TTK has been Provide An Amount @ 1000/Per Day. Please Pay The Total Amount to me i.e. 1000 x 8 = 8000 Rs. And The Outside Medicine And Two Jumbo Packs (Platelets) Which We Arrange On Cash Payment Basis. & The Consultancy Fees Of Dr. Lalit. AT present I have no investigation report and no origin jambo back sleep as I have already submitted all document & jambo pack slip both pack bill with kailash hospital stamp at 10.12.2008 and second time 17.04.2009.you have already spend more time in this matter. Please I hope you will consider own request urgently.

Note: please clarified to me what is the reason of delay in the payment of my claim? If you are fail to pay my claim with in the week time I have been pursue this matter in the consumer court. To avoid the legal step please clear my payment with in the above said period,

we have already send this copy to TTK HEALTHCARE (P) LTD. AND ICICI LOMBARD BUT WE HAVE NOT RESPONSE FOR LAST SIX MONTH. I HAVE VERY TIRED. I HAVE NOT SATISFIED COMPANY SERVICES.PLS DO SOMETHING FOR HELP THIS PROBLEM.
