Dear sir,

i like to inform that i was holding agent license number 7522351143 of icicprulife which was valid

upto 19-3-09.my advisor code is (00130600) ASAP.i have applied for renewal of my license to icicprulife

i understand from icici prulife that my above license was transfer to other insurance co. for which neither

i m interested to transfer my license nor i have applied for. one of the previous employee of icicprulife might have manupilated my license document. i was exclusively working for iciciprulife and wanted to continue for the same
