Not received the New Credit Card

I K Buchibabu, holding Credit Card with your bank. My Credit Card has been expired by 31st December. You people have informed me through phone that you are going to send the New Credit Card to the following address:

Road No. 2, Beside LV Prasad Eye Hospital,

But, I have not received the New Card yet. Kindly look into the matter and treat this very very urgent and do the needful.

My A/c number for your reference is No. 4477 4600 5352 7000 and Date of Birth is 15/ 06/ 1954.

My Mother name is K. Nagarathnamma.
