Delay in Income Tax return

I had filed my Income Tax Return for the year 08 under receipt no.6621003158 dated 31-07. But till date I have not received my refund of Rs.13184/- (Rupees Thirteen thousand one hundred eighty four only) which has been deducted by the State Bank of India, Hari Market, Jammu. From some reliable sources I came to know that the same return has not been filed by State Bank of India, Hari Market, Jammu, to your department, but the bank has given me the Form 16A which has been filed by me with my return . (TDS Challan No.00286 dated 21-02 and BSR No. 0000657 for Rs.13184/-, State Bank of india, Hari Market, Jammu.)
