ISSUE OF NEW ATM CARD AGAINST LOST ONE

Dear Sir/ Mam,

I, Santanu Kumar Dey, a SBSAL A/c holder of Axis Bank (Branch-Saket, New Delhi-17) (A/c No.-160010100065441), have requested for a new ATM card in place of my lost one on 11 Jul 09. But, I have still not received the same.

Hence, I request you to look into the matter to deliver the card at an early date.
