With reference to the above, I beg to inform you that I had send herewith duly filled up form 19 & 10-C on 26.09.08 for release of the above mentioned subject, as a part of my P.F. contribution goes to E.P.S fund in usual course. In this regard please confirm me the status of my claim as the same has not been deposited in to my bank account (B O B A/c no. 08170100004552 Hazra Road branch) nor I have received any information from your side in this regard.

I request your good office and good self to do the needful accordingly so that the claim of the above P.F. should be settled accordingly.
