This is in connection with the above subject mater. In this request I am to state here that I had applied for issuing the Pan card in name of NAINESHKUMAR ASHOKKUMAR KANSARA

But your department has issued me the Pan card bearing No: ADEPK 6946 N

In the name of MAINESHKUMAR ASHOKKUMAR KANSARA instead of NAINESHKUMAR ASHOKKUMAR KANSARA as well as my father name is also mentioned incorrect i.e. ASHOKKUMAR MARATLAL KANSARA instead of

ASHOKKUMAR AMARATLAL KANSARA .

For above mentioned correction of have been reminding frequently

Since the last one year but neither anything is heard from your department in this regard nor corrected Pan card as stated above is issued to me till this date.

As such I once again request you to issued me the correct Pan card at the earliest with out awaiting for any further reminding looking to the importance of Pan card.

Further since my residence address is now changed it is kindly requested to send me the corrected Pan card to the following address.
