Not Received The Cheque Of PF

This is reference to above subject that your concerned department has sent the cheque to my ICICI Bank at Ganesh plaza, kankaria, maninagar, ahmedabad dated 12/12/08.After that i received a second letter from your side for REAUTHORIZATION of PF Account no MH/94095/1449 DATED 19/03/09 said that my cheque no 58394 & 135237 were returned back due to reason- NOT KNOWN.

I fail to understand that bank is there & my account is also with the bank. If it is not possible to deliver than send it at my residential address. It is more than 4 months gone & there is no progress from your side. Kindly requested to look into this matter ASAP.
