Non payment of pension on 1november09

Dear sir,

I had forwarded the 'Existance' certificate through your C.A.B Lucknow office in respect of my policy no 210174966 in the middle of Sep09 and the form forwarded by you for direct NEFT payment to my S/B A/c no 681310110000172 with Bank of Inda Sarojini Nagar Lucknow by post in the middle of october09. However, no payment has been received so far.

You are requested to expedite the same and oblige.
