Lost pan card

I lost my pan card, I lodge a complaint in near by police station and got FIR, and I have a copy of my form 16 which indicates the pan card no and i have a copyof pass port for ID proof. Now I want to apply for duplicate pan card what are the procedures to apply for duplicate pan card? will you please suggest me.
