EPF Office Bangalore Complaints - my epf not yet settled by EPF Office, Bangalore

Dear Sir,

I worked with Les concierges services Pvt Ltd (Head office Bangalore) at Delhi as a Desk executive, i had resigned 24th April and i had completed my resignation formalities & and all the papers send to EPF Office, Bangalore through my Employer. After 3 Month I received my form back by this office and application details are mention below :

Please fill citi bank branch address.

I fill new form and send it back to PF office by my Employer & again after three months i got my Pf form back.application details are as below,

Your account has been already settled.

Kindly look in to the matter and do the needful.
